ODHA/ODMHA On-Line Supervision Form

This form is designed for use to accomodate all supervisors in the branch. Senior Body Officials must use this form to complete their supervision requirement. District and association supervisors may also use this site in situations where their district does not already have an on-line system.

INSTRUCTIONS FOR USE:

• On the title screen, click on "ODMHA" to enter the minor supervision website.

• Ensure that the referee's email address is properly filled in.  If you don't have the official's email address, please input the DRIC's email address.  A list of RIC emails can be found here.

• Ensure that YOUR email address is properly filled in so that you get cc'ed on the resulting email.

• Once you click "Save", the email will automatically be sent to the supervisor and supervisee.
